How does the confirm link work?

Appending ?sub_confirmation=1 to a channel URL makes YouTube show a “Confirm subscription” dialog on load instead of a silent subscribe.

Does the link subscribe people automatically?

No — it opens the confirmation dialog, nothing more. The viewer still clicks Subscribe; the link removes a navigation step, not the choice.

Does it work inside the YouTube app?

The sub_confirmation parameter is a web behaviour. Inside the app the link may just open the channel, so use it where a browser handles the click.
